COVID-19

I have completed a COVID-19 awareness course with the FHT. The FHT is my governing body and is recognised by the General Medical Council. 
 

Prior to the treatment I will send you a questionnaire to complete in relation to COVID-19 health and safety.

On arrival I will take your temperature, and provide an NHS QR barcode to use on the NHS COVID-19 app. As part of the Track and Trace, I will be keeping brief details of your visit for 21 days after your treatment.
